Vaginal Discharge: When Is It Normal and When to See a Doctor?

I often see patients who come to me worried, thinking that any vaginal discharge (safed pani) is a sign of disease. This is one of the biggest myths I have to clear up every single day. Let's get this straight from the start: vaginal discharge is not only normal, it's a sign that your vagina is healthy and cleaning itself.
Think of it as your body's natural housekeeping system. This fluid is made by glands inside your vagina and cervix to carry away dead cells and bacteria. It keeps the area clean and helps prevent infections. So, having some discharge is perfectly fine.
The question isn't if you have discharge, but what kind of discharge you have. The trick is to understand what's normal for your body and when a change signals that something is wrong.
So, What Does 'Normal' Discharge Look Like?
One thing many patients don't realize is that 'normal' isn't just one thing. It changes throughout your monthly cycle, and that’s completely expected. Your hormones are the conductors of this orchestra.
Around ovulation (the middle of your cycle when you release an egg), you might notice the discharge is clear, slippery, and stretchy, a bit like raw egg white. This is your body getting ready for a potential pregnancy (garbhavastha). In the days before and after your periods (masik dharm), it can be thicker, whiter, or even a bit yellowish and sticky. All of this is part of a healthy rhythm.
The amount can vary too. You might notice more if you're exercising, sexually aroused, or taking birth control pills. It's your body's response to different situations. No two days are always the same.
The Red Flags: When Discharge Signals a Problem
So how do you know when it's time to stop guessing and consult a qualified healthcare provider? In my experience, your body gives you pretty clear signs. You need to pay attention to sudden changes from your personal normal. Here are the things to watch out for:
- A major change in colour: If it turns grey, greenish, or a prominent yellow.
- A change in texture: If it becomes frothy or clumpy, sometimes described as looking like paneer (cottage cheese).
- A strong, unpleasant smell: Especially a fishy or foul odour that is noticeable.
- Associated symptoms: This is a big one. If the discharge comes with itching (khujli), burning, redness, or soreness around the vagina.
- Pain: Any pain in your lower belly or pelvis, or burning when you urinate.
Common Reasons for Abnormal Discharge
If you're experiencing any of those red flags, it's often due to an imbalance in the delicate environment of the vagina. This can lead to a few common infections.
One is Bacterial Vaginosis (BV), which happens when there's an overgrowth of certain bacteria. It typically causes a thin, greyish-white discharge with a distinct fishy smell. Another common culprit is a yeast infection, or Candidiasis. This one usually gives you a thick, white, lumpy discharge with intense itching and burning. Less common, but important, is Trichomoniasis, a sexually transmitted infection that can cause a frothy, yellow-green discharge.

Trying to self-diagnose can be difficult because symptoms can overlap. Using over-the-counter creams without knowing the cause can sometimes make things worse. It's always a good idea to get a proper diagnosis.
Your Lifestyle and Vaginal Health
Your daily habits can play a role in maintaining a healthy vaginal environment. Especially here in India, with our hot and humid weather for many months of the year, a few simple things can make a difference. Wear breathable, cotton underwear and avoid very tight synthetic pants for long hours. This helps keep the area dry.
Please, do not 'douche' or wash inside your vagina with soaps or antiseptic liquids. Your vagina cleans itself. Using these products washes away the good bacteria and can disrupt the natural balance, making you more prone to infections. Just gentle washing of the external area with plain water is enough.
And no, eating dal, roti, or a little ghee won't cause an infection. A balanced diet is good for your overall health, which includes your vaginal health. Some people find that including yogurt (dahi) in their diet helps, but it is not a cure for an active infection.
What to Expect at Your Doctor's Visit
I know it can feel embarrassing to talk about this, but please don't. We see this every single day. I recently had a 28-year-old software engineer from Gurgaon visit me. She was extremely stressed about a project deadline and noticed a change in her discharge along with some itching. She was worried it was something serious, but after a simple examination, we found it was a straightforward yeast infection, likely triggered by stress.
When you come in, I'll first ask you some simple questions. What do you see? When did it start? Does it smell? Do you have any itching or pain? I'll also ask about your periods and sexual history. This helps me understand the full picture.
Sometimes, a physical examination is needed. I might take a small sample of the discharge with a cotton swab to look at under a microscope or send for a lab test. It's a quick and usually painless process. This helps confirm the exact cause so we can give you the right treatment. There is nothing to be afraid of.
