Health Insurance Claims and Portability in India

Everything about a health policy is theoretical until a claim is made. That is when the clauses start doing work: who authorises a cashless admission and how fast, which documents a reimbursement needs, what the insurer is allowed to deduct, and what happens when a claim is refused.
These guides cover the claim stage and the exit route. Two things here are rights rather than favours, and most policyholders do not know they have them: IRDAI-mandated turnaround times on cashless authorisation and settlement, and the right to port to another insurer while carrying accrued waiting-period credit with you.

Frequently Asked Questions
How does a cashless claim work in India?
The hospital raises a pre-authorisation request with the insurer or its third-party administrator, which approves an amount before treatment, then a final approval at discharge. IRDAI now imposes turnaround times on both steps. Our guide covers the workflow, the documents involved, and when reimbursement is the only route.
Why do health insurance claims get rejected?
The recurring grounds are documented and largely avoidable: an undeclared pre-existing condition, a claim inside a waiting period, a permanent exclusion, non-payable consumables, or late intimation. Insurers must give reasons in writing, and there is a defined escalation path through the grievance cell, IRDAI and the Insurance Ombudsman.
Can I change my health insurance company without losing my waiting period?
Yes, through portability, provided you apply within the notice window before renewal and maintain continuity. Accrued waiting-period credit carries over up to your previous sum insured — the common surprise is that credit generally does not extend to the incremental cover if you increase the sum insured at the same time.
