Routine blood work is the most common thing a CGHS beneficiary is sent for, and every test on the prescription has a notified rate that an empanelled laboratory has to bill against.

The tables below cover the tests that appear on most prescriptions — complete blood count, blood sugar and HbA1c, lipid profile, liver and kidney function, thyroid, vitamin D and B12, and the cardiac and clotting markers ordered in an emergency.

Rates shown are as per the CGHS approved package rates (Office Memorandum F.No.5-16/CGHS(HQ)/HEC/2024(PartI) dated 03.10.2025, effective 2025-10-13) (2025 (effective 13.10.2025)) and are subject to periodic revision. Always verify the latest approved rates on the official source.

What Decides Which Column You Are Billed At

Every laboratory test carries two figures: a non-NABH rate and a NABH rate that is about 15% higher. An accredited laboratory bills the higher figure for the same test, so the same HbA1c can be Rs 255 at one empanelled centre and Rs 300 at another, and both are correct.

A panel is almost always cheaper than the tests inside it ordered one by one. A complete haemogram is Rs 255 (non-NABH), while haemoglobin, platelet count, ESR and a peripheral smear billed individually add up to more. The same applies to the kidney and liver function panels.

How CGHS Package Rates Work

A CGHS package rate is an all-inclusive amount covering admission, surgeon and anaesthetist charges, operation theatre, nursing, routine investigations, medicines and accommodation for the standard length of stay. For an investigation, the rate is the whole charge for that test at an empanelled facility.

The 2025 memorandum lists three columns for every code: a non-NABH rate, a NABH rate that is about 15% higher, and for some categories a further super-speciality rate. Which column applies depends on the accreditation status of the hospital or laboratory you are treated at, not on your ward entitlement.

High-value implants, stents, lenses and devices are not part of the package. Where a code says the rate excludes an implant or a drug, that item is reimbursed separately against its own notified ceiling.

Referral and Prior Permission

For planned (non-emergency) procedures, a CGHS beneficiary needs a referral from the CGHS wellness centre and prior permission from the competent authority. Emergency treatment at an empanelled hospital is reimbursed up to CGHS rates without prior permission.

For investigations, the test has to be prescribed by a CGHS medical officer or by a specialist at a government or empanelled hospital for the empanelled laboratory to bill it as a cashless CGHS test.

  • Carry a valid CGHS card and follow your ward entitlement (based on pay or pension)
  • Use an empanelled hospital or laboratory for cashless treatment up to CGHS rates
  • Obtain prior permission and a referral for planned surgery
  • Ask for the OM code the hospital is billing against, and check it against the rate list

These Rates Are Revised

CGHS revises its package rates periodically, and the memorandum in force on the date of your treatment is the one that applies. The figures on this page are from the memorandum effective 13 October 2025. Before you plan a procedure, confirm the current figure for your code on the official memorandum linked above.

Frequently Asked Questions

What is the CGHS rate for a CBC blood test?

Under the memorandum effective 13 October 2025, a complete haemogram (CBC with Hb, RBC indices, TLC, DLC, platelets, ESR and peripheral smear) is Rs 255 at a non-NABH facility and Rs 300 at a NABH facility.

Do CGHS beneficiaries pay for blood tests at an empanelled laboratory?

Listed investigations are cashless up to the CGHS rate at an empanelled laboratory when they are done on a valid CGHS prescription or referral. If you get the test done outside the empanelled network, reimbursement is limited to the CGHS rate.

What is the CGHS rate for a thyroid test?

A thyroid function test covering T3, T4 and TSH is Rs 383 (non-NABH) and Rs 450 (NABH). TSH alone is Rs 170 and Rs 200 respectively.

Is vitamin D covered under CGHS?

Yes. A vitamin D3 (25-hydroxycholecalciferol) assay is listed at Rs 850 (non-NABH) and Rs 1,000 (NABH). It has to be prescribed by a CGHS medical officer or an empanelled specialist to be billed as a CGHS test.

Why is my laboratory charging more than the CGHS rate?

An empanelled laboratory cannot charge a beneficiary above the notified rate for a listed test. If it does, ask which OM code it is billing against and raise the difference with the CGHS wellness centre. Non-empanelled laboratories are not bound by these rates.
